About the Client

Medi Route

Medi Route is a healthcare coordination product spanning Primary Care Networks. Admins oversee the full system. PCNs add and assign GPs, pharmacists, CHAs, and patients, and monitor adherence and risk across their network. GPs review assigned patients, create prescriptions, assign CHAs, and track status with patients and pharmacists. Pharmacists process and deliver medication, which starts reminder notifications for adherence. CHAs visit patients at home to provide care and ongoing support.

What we Delivered

A complete multi-role care platform: mobile for patients and CHAs; web for Admin, PCN, GP, and Pharmacist. Networks are managed centrally, prescriptions flow to pharmacy and patient, adherence reminders kick in after delivery, and CHAs support patients at home.
