Medi Route Healthcare Platform
Medi Route connects six roles across primary care: Admin, PCN, GP User, Pharmacist, Patient, and CHA. Patients and CHAs use mobile apps; everyone else works on the web. The platform covers network management, prescriptions, pharmacy delivery, adherence reminders, and home visits.
Medi Route
Medi Route is a healthcare coordination product spanning Primary Care Networks. Admins oversee the full system. PCNs add and assign GPs, pharmacists, CHAs, and patients, and monitor adherence and risk across their network. GPs review assigned patients, create prescriptions, assign CHAs, and track status with patients and pharmacists. Pharmacists process and deliver medication, which starts reminder notifications for adherence. CHAs visit patients at home to provide care and ongoing support.

Challenges We Overcame
Separating Admin, PCN, GP, Pharmacist, Patient, and CHA journeys across mobile and web without overlapping permissions or confusing workflows.

What we Delivered
